Pacific Homecare seeks a full-time Placement Coordinator responsible for supporting our Central Valley District by assessing each family's care needs to match our Care Provider's experience and availability. This position actively creates placements and follows up with families to ensure they are receiving high quality respite service.
Primary Responsibilities include but are not limited to the following:
· Create placements between families and Care Providers
· Schedule Parent Orientations for all new referrals
· Maintain confidentiality of our clients and providers in compliance with HIPAA
· Complete follow up calls to Clients and Care Providers
· Assist with DOJ Fingerprint Rolling
Education and Experience:
High School Diploma with 2+ years of experience
Excellent written and verbal communication skills
Must be computer savvy and proficient in Microsoft Word, Excel, and Outlook
Bilingual in Spanish/English is preferred but not mandatory
